What is PCORI HSII?

The Patient Centered Outcome Research Institute (PCORI) has launched the Health Systems Implementation Initiative (HSII) with an initial investment of $ 50 million for promoting the use of comparative clinical effectiveness research in healthcare. A part of the funding for this initiative may be siphoned from the PCORI Fee charged on self-insured health plans. Through the HSII, PCORI aims to provide funding to HSII Participant Health systems for undertaking projects to actively advance the adoption of new evidence.

The PCORI HSII has a specific goal of promoting lasting change within participating systems and laying the groundwork for broader adoption of evidence based practices. HSII includes a total of 42 health system participants. The participants list was announced by PCORI in March 2023. This list includes 

Academic Medical Centers, 

Community-Based Systems, 

Safety Net Health Systems, 

Integrated Healthcare Delivery and Finance Systems, 

Public Healthcare Delivery Systems, 

Faith Based Systems, and 

a medical center within the Veterans Health Administration.



HSII Implementation Projects

What are the different types of projects implemented via the HSII? PCORI announced a $37 million funding in October 2024 for the first set of HSII implementation awards. A total of 24 HSII Implementation Projects were included in this list. A HSII Pilot Project was also included in the initial implementation set. The first implementation set was primarily focused on either one of the 2 topics based on evidence from PCORI’s research portfolio. The topics were -


  1. Intensive Lifestyle Treatment for Weight Loss in Primary Care Settings

  2. Appropriate Antibiotic Prescribing for Children with Acute Respiratory Tract Infections (ARTIs).


How is the funding obtained for PCORI HSII? PCORI issues calls for proposals for BSII Implementation Projects to raise funds for these projects. This way all HSII participants are provided with an opportunity to propose projects that promote the uptake of specific PCORI funded evidence within their healthcare delivery settings. New calls for proposals are issued by PCORI on an annual basis.


PCORI HSII Learning Network

With the HSII being a research oriented initiative, having a solid learning network is essential for its smooth functioning. The HSII Learning Network provides a venue for HSII participants to connect with each other and with PCORI as they undertake various projects for Implementation and Capacity-building. All the 42 participants identified and added by PCORI are part of the HSII learning network. 


PCORI HSII participants can use the learning network to meet and engage with each other, access support such as Office hours with investigators from research studies funded by PCORI. They can even help each other via the network by offering suggestions and inputs to PCORI. One example would be suggestions on potential evidence topics that would be the main focus of the implementation.
